Authorities say Haleem Samad Deloach, who would now be 45 years old, went missing on April 12, 2015, after being taken to Clara Maass Medical Center in Belleville.

Police had found Deloach sitting on a park bench and brought him to the hospital, but according to relatives, they have not seen or heard from him since.

Deloach is described as a Black male with a medium complexion, standing 6 feet 1 inch tall and weighing about 200 pounds.

He has short black hair and brown eyes. At the time of his disappearance, it is not known what clothing he was wearing, and no distinguishing marks have been identified.

His case has been classified as endangered due to his disability, raising concerns about his well-being.

The New Jersey State Police Missing Persons Unit continues to list his case under file number H660202500065.
