The Sickle Cell Association of NJ will off a four-day Certification Seminar for chronic disease self-management program (CDSMP).

**Get Certified to Lead:** Knowledge is power! Thanks to a grant from the State of New Jersey, and support from people like you, the Sickle Cell Association of New Jersey (SCANJ) is able to serve as a host agency for the 4 Day Peer Leader Training Certification Course and for 6-session community workshops for the Take Control of Your Health Program.

**Who Should Attend:** Adults, Health Care Professionals and others, who have, or care for someone with, sickle cell disease, diabetes, high blood pressure, obesity, arthritis, asthma or other chronic health problems. Any adult who wants to volunteer to help others to a healthier life through this evidence-based, successful program.

**Why You Should Attend:** It works! Studies have shown that the Take Control of Your Health Workshop produces positive results for people who complete the program. As a certified Peer Leader volunteering with SCANJ, you can help yourself and others to a healthier life. Participants report the following benefits: Improvements in physical activity, stamina and pain control:

Better communication with their health care providers

Fewer doctor and hospital visits

Fewer emergency room visits

You’ll learn how to make better nutrition and exercise choices, relaxation techniques to help reduce your stress, better ways to communicate with your doctor and loved ones about your health, and more.

The workshop was developed by the Stanford School of Medicine Patient Education Department and has been held at locations throughout the country and around the world. SCANJ’s workshops are led by state-certified Peer Leaders, some with health conditions themselves. The workshops are under the supervision of SCANJ Master Trainers.

Workshops are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.
